Mid-Year Report

You will need to fill out a mid-year report to update us about your progress during the first six months of your program participation. The mid-year report includes areas to report on activities related to:

• Awareness Resources
• Health Communications Campaigns
• Prevention and Health Education Screenings
• Behavior Change / Disease Management Programs
